While there may be numerous reasons for blindness, all involve the degeneration of the cells that pick up light in the eye, but in the new study, the researchers were able to repair the damage done to these photoreceptors using stem cells — which experts are calling a “significant breakthrough” and a “huge leap” forward.

Researchers from Moorfields Eye Hospital and University College London conducted tests in animals in which they were able to repair the photoreceptors in eyes by completely replacing them using stem cells. It had previously been shown that stem cells could be used to replace the “support” cells within the eye that help keep the photoreceptors alive, but the present research has gone a step forward and made significant progress.

For the present study, the researchers actually created retinas in the laboratory, collecting from them thousands of stem cells, which were then reprogramed into developing into photoreceptors. These were then injected into the eyes of blind mice, and gradually, these new photoreceptors were incorporated into the blind eyes, going on to begin function and restoring, if at least partially, eyesight in the mice.

Speaking about the research, Dr. Robin Ali, who led the study, said, “This is a real proof of concept that photoreceptors can be transplanted from an embryonic stem cells source and it give us a route map to now do this in humans. That’s why we’re so excited, five years is a now a realistic aim for starting a clinical trial.”

The researchers did add though that the effectiveness of such a treatment was still “low,” as out of a transplant of around 200,000 photoreceptor cells, only about 1,000 eventually “hooked up,” with the blind eye giving partial vision.

Commenting on the study, University College London Prof. Chris Mason said, “I think they have made a major step forward here, but the efficiency is still too low for clinical uses. At the moment the numbers of tiny and it will take quite a bit of work to get the numbers up and then the next question is ‘Can you do it in man?’ But I think it is a significant breakthrough which may lead to cell therapies and will give a much expanded knowledge on how to cure blindness.”

TORONTO – Pat Sedgwick awoke earlier than usual on an August morning last summer to a rolling curtain of black dots blocking the vision of her right eye.

A 79-year-old from Toronto, Sedgwick had been anticipating a rare day of complete solitude in her P.E.I cottage. But that’s not the way the day unfolded.

“It was frightening,” she recalls.

“I couldn’t identify what I was seeing. I was just seeing a very mixed-up kaleidoscope. And there were round little circles and they’d go up and down and then they’d go away.”

Sedgwick called her daughter, who assured her it was merely a symptom of aging, or a result of the cataract surgery Sedgwick underwent three years earlier. Sedgwick didn’t mention the little intermittent dots.

But as a precaution, her neighbour drove her to the hospital, where she was informed her retina had detached. It’s an ailment that, if not treated in time, can result in complete blindness in the affected eye.

Dr. Netan Choudhry, director of vitreoretinal surgery at Toronto’s Herzig Eye Institute, says retinal detachment affects less than one per cent of people.

That figure rises to three per cent if people live to be 85 or older, according to a 2008 British Medical Journal study. The condition is more common in those with severe short-sightedness, people who have had cataracts removed, or those who are diabetic, as Sedgwick is.

Despite the low prevalence, Choudhry says a detached retina remains a “complicated condition” with only one right course of action: Seek immediate medical attention, ideally from an eye specialist.

“Retinal detachment is an emergency, and should you delay, it could result in a poorer prognosis in the treatment…. Invariably, time is often of the essence,” he says.

The retina is a layer at the back of the eyeball which contains photoreceptor cells. They receive light and convert it into signals that the brain processes as vision.

“The retina functions like a camera film, and basically it’s the reason we’re able to have sight,” Choudhry explains.

When it detaches — through aging, trauma or a pre-existing retinal condition — the gel that sits in front of the retina breaks down and separates, pulling on the retina and eventually peeling it away. Once it does, the gel can get underneath the retina and cause a complete separation of the retina from the back of the eye.

When that happens, people can suffer sudden loss of vision, the appearance of flashing lights, thunderbolts, or floaters — tiny spots in vision, which are often blood cells that have dispersed in the eye — or a big dark shape blocking part of the vision from the affected eye.

Choudhry says there is almost never pain associated with the detachment, unless it occurs as a result of a trauma.

Reattaching a retina now carries a success rate of 80 to 95 per cent, depending on the level of scarring and how long it’s been detached, Choudhry says. There are many operations available to try and reattach it, most commonly the injection of gas or silicone to absorb the gel that caused the separation.

But all the procedures aim to do the same thing, which is to seal up the tear, and they need to happen quickly before the macula detaches as well. The macula, located in the middle of the retina, is the part responsible for central and high-quality vision. Once it detaches, the chance of restoring vision is much lower.

Even if the macula remains attached, surgery must happen within 72 hours. That’s because those photoreceptor cells that convert light into signals for the brain lose access to oxygen when the retina detaches, and they start to die. While there is ongoing research aimed at finding ways to protect these cells in the event of retinal detachment, the medicine is not there yet, Choudhry says.

“If the macula is attached, then you have a huge potential for recovery, because that central portion of vision is still good, so you better get going quickly,” says Dr. Guillermo Rocha, an ophthalmologist based out of Brandon, Man.

Prevention is the only true tool against retinal detachment, with at least annual visits to your eye doctor. Once it happens, though, there are no home remedies or steps you can take to mitigate the damage. You need to get to medical care.

Rocha, the lone full-time ophthalmologist in the Prairie Mountain Health Region, knows how difficult it can be to access that necessary immediate treatment. He’s had patients who have been unwilling to make trips in for care because of distance or bad weather.

“Honestly, I don’t accept that,” says Rocha. “I don’t care if you have to take a bus, but you have to do it if you want to preserve your vision.”

He says that for retinal detachments among rural citizens, awareness of the urgency of the situation — and who to see about it — is even more crucial.

“Some might go to emergency, they wait for four hours, and already they’ve lost time just waiting,” says Rocha. “The sequence of events should be to see an ophthalmologist immediately.”

The good news, says Rocha, is that the field has advanced significantly, even over the two decades he’s been practising. Once thought impossible to repair, laser technologies and developments in treatments have given surgeons and ophthalmologist more tools to work with.

“Retina surgery is a bit like neurosurgery, in the sense that I remember when I first went into medical school, they would say, once they opened the skull, that’s it for the brain, there’s no success,” he says. “Slowly, obviously, we’ve seen that’s not the case. It’s the same with retinas.”

And in the more remote areas of northern Alberta and Manitoba, telemedicine has become part of the process. Rocha says using video conferencing to diagnose conditions and identify whether or not a person needs to be flown to an urban centre for care cuts down on critical travel time.

For her part, Sedgwick was unable to get the surgery she required in P.E.I. and so had to be driven to Halifax, where she had one procedure. She had a second clean-up surgery after her return home to Toronto.

Her recovery has been steady, and she now says that her vision is much improved. She acknowledges that she’s fortunate, given her circumstances, but knowsshe’s at higher risk of having a repeat episode, especially in the other eye.

“They can’t promise (it won’t happen again),” she says.

But she added, laughing: “I can promise one thing — if I see a black dot I’m going to go immediately. I’m not fooling around anymore.”

MONDAY, July 1 (HealthDay News) — Collapsible laundry hampers can cause serious eye injuries to children if a sharp wire contained within the device breaks free, according to a new report.

The researchers documented the cases of two children, one 23 months old and the other 11 years old, who each suffered a puncture wound in one eye from a collapsible laundry hamper.

The devices collapse and then pop back into shape because they have embedded within them a flexible wire that winds around the outside of the cloth hamper.

“The wire has fabric holding it in place, and it’s like a humongous spring,” said study co-author Dr. Iris Kassem, an assistant professor of clinical ophthalmology at the University of Illinois at Chicago’s School of Medicine. “When the fabric becomes frayed, the wire pops out and the end of the wire is very sharp.”

The 11-year-old boy suffered a corneal laceration while placing clothes in a collapsible laundry hamper, according to the report. The wire mechanism within the hamper suddenly snapped up and struck his right eye, puncturing it.

The report said the 23-month-old girl received her injury after being poked in the eye from a wire protruding from a collapsible hamper.

Both patients came to the University of Illinois at Chicago Eye and Ear Infirmary for treatment within one year of each other.

The cases were detailed online July 1 and in the August print issue of the journal Pediatrics.

These types of penetrating eye injuries are uncommon but very serious, said Dr. Alon Kahana, an oculoplastic surgeon at the University of Michigan Kellogg Eye Center.

“The risk of vision [loss] is acute, and those patients require immediate evaluation in an emergency room,” Kahana said. “Outcomes can be very good. There are some patients that end up with 20/20 vision, [but] there are some patients who end up with no vision at all.”

In both reported cases, the children received prompt emergency treatment and, as a result, are expected to regain much of the sight in their injured eyes, Kassem said.

Both children required eye surgery to repair the damage. They have since required some vision therapy to fix developmental problems that occurred as a result of temporarily losing sight in one eye at such a young age. The boy has suffered from exotropia, a form of crossed eyes, while the girl has had amblyopia, or lazy eye.

“They both did extremely well,” Kassem said. “They both got very lucky. They both beat the odds to do well.”

The authors have reported the injuries to the U.S. Consumer Product Safety Commission, Kassem said.

They urged parents to be aware of the risk to kids. “Children shouldn’t be playing around these things, and if the integrity of the hamper is compromised in any way, you need to throw the product away,” Kassem said.

Kahana agreed. “When it pokes out, it pokes out with force. You have the combination of sharp and force,” she said. “Most of these are cheap items not meant for extended use. People should see when something approaches the end of its useful life and toss it away.”

Kassem has two kids and, at the time of the injuries, had a couple of these hampers around her house.

“When the second kid came in with an injury, I said, ‘That’s enough of that,’ and got rid of the hampers,” she said. “It kind of freaked me out.”

Still, I coped fairly well up until the past year, when the cataract slowly took the remaining vision in the eye and with it the enjoyment of many things. It still wanted to see and wanted to be the dominant eye, so seeing anything was frustrating, the only clear vision I had in that eye was without the contact lens, about a half inch from my eyelashes- great superpower to have if you want to examine diamonds for flaws, but beyond that all was a blur.

Five days before Al and I were wed, things started to go wrong in my left eye- flashes of light, blurry spots moving around and a big change in the number and type of floaters in it. I was really afraid of another detached retina, possibly more surgery and further loss of vision, but it hasn’t detached yet and may not for a while yet. The retina specialist I see yearly recommended seeing about an operation to help me regain some vision in the right eye. Exciting news for me!

So, after a series of doctors’ appointments to get the ball rolling, this past Wednesday I was wheeled into an operating theater and had my old, worn out and foggy lens removed from my eye and a brand new crystal clear lens implant put in.

What a difference! How I see without any glasses or contact lens for my right eye.

Immediately, even though I had a clear eye protector over my eye, I could see better! And without any corrective lens of any kind. At my follow-up appointment yesterday, the cover was removed, and this morning I didn’t need my glasses to see my nightstand clock. Five years of frustration are over, and I look forward to discovering new scenery every day and seeing what I have missed! I have the feeling the fireworks are going to be spectacular this year!
