This is taken from another thread I posted in and didn’t get any responses. The thread was complaining
about the lack of training and understanding being exhibited by many of today’s opticians.
let’s see how many answers I get to this question. I have used this in the past about 25% knew the answer.
which of the following items can you use to find the optical center on a lens the two lenses in the list are not the lens you have to find the center

1 pencil
2 marking pen
3 piece of paper
4 +1.00 sph
5 -1.00. Sph
6 caliper
7 microscope
8 mm scale

Troubleshooting guidance for a potential progressive non-adapt

Hi all, I’m hoping someone can give me guidance (as I’ve exhausted my IRL brains trust on this one).

I have a px, an late 50s investment advisor who sits on his PC all week, goes acrobatic aircraft flying on weekends, and does photography in his spare time.

He has been wearing 2 SV pairs (SVD and SVI) for the past year.

Given Rx (old SVD): R -525/-100×129 L -400/125×067
Given Rx (old SVI): R -350/-100×129 L -225/-125×067
Our PAL: R -450/-100×130 L -400/-125×073 Add +175

The old SVs were completed without heights accounted for. A vertical decentration of approx 10-11mm exists in both lenses.

Verting his SVD at actual pupil as worn showed prisms of R 6.0D BU and L 3.6D BU, with some horizontal prism of about 0.40D BO.

He’s become used to these despite the prism and, what I assume would’ve been initial slight distortion with being off-centre.

We ended up making him a pair of PALs non too dissimilar to the Varilux X, in a frame shape almost identical to his SVD pair (base curves match, oval/rectangular shaped, next to no decentration). He is not adapting and is complaining of swim effects to the point of vomiting. POW parameters, etc have all been checked and cleared.

Can’t seem to troubleshoot this one. Would his VI on his old pairs be ‘throwing off’ the numbers so to speak?

Offset round seg complicated rx…freeform magic possible???

Jacqui was the Franklin queen. Is she still around?

I know Three Rivers used to do a nice digital backside round seg, but I don’t know if they can put the distance optics where you need them with a 10mm temporal decentration if they have standardized, inflexible software. It is an old design, but maybe updated? (That assumes there is a non-standardized, flexible software that can float the distance optics anywhere.)

I will say, though, that if she’s a binocular vision disaster, you may want to trial frame (including prism) that bad boy to see what happens to her fusion in that direction of gaze. There are conditions that require different prism in different directions of gaze (impossible to deliver, of course), so make sure this “eye turning vs. head turning” exercise doesn’t lead to futility.

Special ordering frames

Say a patient wants “green frames” and you have zero (happened to me yesterday). So you go to your catalogues and point out several that are candidates and the patient approves.

What is your policy/procedure?

Order a max number of frames, like three?

Charge any “special ordering” fee?

Or, do you operate your optical such that you discourage special ordering, and stick to what’s on-hand?

Do you do something in-between…for example you will find a frame style in inventory that looks good and fits well, and just order in another color?

Flummoxing PAL fit

Hi everyone, fresh optician seeking some advice.

The following patient has been wearing Hoya’s ID lifestyle 2 for that past 5 years and loves it, they can do everything they need to. They recently came in with a new RX which shows minimal change (OS went from +0.25-0.75×160 to PL-0.50×160, OD the same at -0.25-0.25×130, add still +2.50).

I use Essilor not Hoya at the shop, so I fit them with Physio 3.0 keeping in mind that the patient wants to wear them all the time, but specifically for computer and reading. Since pick up they have given them an honest try (wore them for 2 weeks) and came back saying that the intermediate FoV is too small. Driving is good, as is close up reading, but for arms length work the “zone” is too small.

Increasing panto and fitting them closer to their face hasn’t provided enough of a change for them to be happy. Meanwhile, when they put on the old ID Lifestyle lenses, they do not have this issue. Both frames are similar in size, within a 2mm of A and B measurements.

Is this an issue of PAL design or am I wrapped up in my own head and overlooking something?
